Sumner County employees and dependents are highly encouraged to attend one of the below HRA events and Health Coaching Classesto fufill the Wellness Promise. Members who do not complete the HRA and subsequent follow-up will be dropped to the Standard Plan.
If your HRA results are within normal range, you do not need to schedule a follow-up appointment or attend a health coaching class.
View your compliance status under Health Tracker > Reminders or email email@example.com. Individual lab results can be found under lab/diagnostic reports. A health coach will be in contact with you to review your results and answer any questions.
Patients who meet any of the below, abnormal metrics must schedule a follow-up appointment and/or attend health classes: Board of Education: LDL > 130, BMI>30, A1C> 7. Blood pressure> 140/90, Nicotine positive Government: Total Cholesterol >220, BMI> 30, A1C>7, Blood Pressure> 140/90, Nicotine positive
