2018 Sumner County Employee HRA Event
Please review the following important instructions below.
In an effort to provide outstanding healthcare and support to you and your family, we have added additional HRA (Health Risk Assessment) Events for Sumner BOE employees for 2018. The entire 2018 schedule is listed below.
IMPORTANT NOTES:
1. Appointments are required for all HRAs. To schedule an appointment, please call One to One Health at 855-571-4500 then press Option 1 for scheduling.
A. If you have already scheduled an HRA appointment and would like to change the existing appointment
please call One to One and they will be happy to help you.
B. Please note that all HRAs must be completed during one of the scheduled HRA Event days and cannot be
be completed outside of the scheduled HRA dates/times.
2. Once your appointment is scheduled, it is recommended to complete the online Consent Form (BOE and GOV employees) and Wellness Promise Form (BOE employees only) or Wellness Incentive Form (GOV employees only), (Please follow the steps below). You may also complete the form(s) on-site. However, completion online will enable you to move faster through the HRA process.
1. Visit www.sumnercountyhealth.com
2. Click "Patient Portal" (at the top of this page)
3. Click "Get Started"
4. Sign in using your username and password (If you do not remember your log-in please click "Trouble
logging in" and follow the instructions)
5. Under the "Upcoming Appointment" you will click "FILL QUESTIONNAIRE", this is the HRA Consent Form
and Wellness Promise form (There are 2 online forms to complete). These will need to be completed
in full. (See image below.)
6. Once the form is complete, click "Submit Request" at the bottom. This will complete the online portion.

2018 Wellness Promise Requirements are for Sumner BOE employees only. Please review the important notes below.
IMPORTANT NOTES:
1. ALL employees currently enrolled in the Wellness Plan for 2018, as well as those wanting to enroll in the Wellness Plan for 2019, must complete the Wellness Promise requirements for 2018.
2. Employees who do not complete their Wellness Promise will be considered non-compliant and ineligible for the 2019 Wellness plan option.
3. The 2018 Wellness Promise requirements include:
A. Completing an HRA with One to One by the April 13th deadline.
B. If you are non-compliant based on your HRA screening you will be contacted by One to One and will be
given more specific information on how to complete the coaching requirements (if applicable).
C. Please note if you are considered "Compliant" on your HRA, a follow-up appointment and/or coaching
with One to One will NOT be required.
4. Under extenuating circumstances employees may receive pre-approval from the BOE Benefits Office to use an outside provider for their HRA (instead of One to One). If you would like to use an outside provider, please call the Benefits Office at 615-230-6952 for approval.
